This English version is based on the original Italian article. For wording nuances, you can refer to the Italian version.

## In short, for parents
- • Some elbow fractures in children need closer attention than they first appear.
- • How much the bone is out of place initially is a key sign of future problems.
- • We need to look carefully at the break itself to predict how the elbow will heal.

A study published in 2026 in the *Journal of Children’s Orthopaedics* followed 140 proximal radius [fractures](https://ortopediaevolutiva.com/en/conditions/injuries-fractures) in 138 children and sought to understand which initial characteristics were associated with complications and less favorable outcomes over time [1].

The minimum follow-up was 5 years, and for re-evaluated patients, the median reached almost 7 years [1]. This case series is therefore useful for looking beyond just initial radiographic healing.

A primary displacement of at least 3 mm was associated with a higher risk of complications, with an odds ratio of 6.7 [1]. Physeal involvement and a higher Judet class were also associated with a less favorable course. These are elements that help distinguish fractures with a more benign prognosis from those that warrant greater attention.

The choice between immobilization, closed reduction, internal fixation, or open reduction remains important, but the study suggests that a significant part of the prognosis is already linked to the initial severity of the injury [1].

Worse functional outcomes were more frequent in surgically treated cases [1]. This data does not demonstrate an unfavorable effect of surgery: in a retrospective study, operated children are likely those with more severe, displaced, or unstable fractures upon presentation. This is the classic confounding by indication.

In the initial assessment, I would therefore pay particular attention to the actual displacement, physeal involvement, and Judet classification. These elements can influence both treatment and how we set up follow-up and counseling.

The study does not provide an automatic threshold for surgery. Rather, it identifies initial characteristics associated with a higher risk, which must be integrated with age, stability, possibility of reduction, and clinical picture.

The study is retrospective and from a single centre, with not entirely standardized protocols and a proportion of patients lost to follow-up [1]. However, the number of cases and the duration of observation remain strengths for a relatively uncommon injury.

Initial displacement, physeal involvement, and Judet class therefore help define the risk already at the first evaluation. Two fractures, both described as "proximal radius fractures," can have a very different course.
