This English version is based on the original Italian article. For wording nuances, you can refer to the Italian version.

## In short, for parents
- • Rapid MRI can define the location and extent of infections in children’s bones, joints and muscles.
- • At the study centre, the shorter protocol reduced scan times, sedation and contrast use, length of hospital stay and hospital costs: it does not establish fewer admissions or lower expenses for every family.
- • It is useful when it resolves clinical uncertainty; it must not delay urgent treatment that is already indicated.

Magnetic resonance imaging is often the most useful examination for defining the location and extent of a pediatric musculoskeletal infection. The limitation, especially in emergencies, is organizational: waiting times, examination duration, contrast, and sedation can slow down a process that should remain rapid. [1][2]

**Rapid MRI** protocols aim to reduce precisely these obstacles. They use a limited number of sequences, generally without contrast, and with the goal of avoiding sedation when possible. [1][3]

## What Changed in Chan's Study

Chan and colleagues compared the traditional pathway with a rapid MRI protocol in children evaluated for acute musculoskeletal infection. [1]

With the new protocol, sedation decreased from 53% to 4%, and contrast use from 88% to 0%. The median time from request to examination dropped from 6.5 to 2.2 hours; the MRI duration from 63.2 to 24 minutes. The time to final report, length of hospital stay, and hospital costs also decreased. [1]

These are interesting data because they concern the entire pathway, not just image quality.

## Where It Can Be Particularly Useful

The advantage is greater in children where the location and extent of the infection are not already clear: suspected osteomyelitis, pelvic infections, deep myositis, possible abscess, or [septic arthritis](https://ortopediaevolutiva.com/en/conditions/septic-hip-arthritis) with doubtful bone involvement.

In these situations, an early available MRI can show whether the process is localized, multifocal, or associated with collections requiring drainage. A systematic review from 2024 highlighted the potential of early MRI in better defining the extent of the disease and guiding initial treatment. [2]

## When Waiting for MRI Can Be a Mistake

A rapid protocol should not become a mandatory step for every suspected infection.

If the child is septic or the picture of septic arthritis is already very convincing, waiting for an examination that will not change the decision can delay aspiration, drainage, or antibiotics. Rapid MRI is useful when it resolves a clinically relevant uncertainty; it has much less value if the therapeutic decision is already clear.

## The Real Limit Is Often Organizational

An international survey published in 2025 found rapid MRI protocols for acute musculoskeletal infections in only 22% of responding institutions. Pediatric centers used them more often. The main barrier reported was not technology, but the absence of a shared protocol. [3]

For it to work, the pathway requires agreement among the emergency department, orthopedics, radiology, anesthesia, and pediatrics: who selects patients, which sequences are performed, when contrast is needed, when it is acceptable to forgo sedation, and with what priority the examination enters the radiological schedule.

## What Changes in Practice

Rapid MRI does not replace conventional MRI in every situation and does not eliminate clinical judgment. However, it offers a concrete solution for quickly obtaining information that, in some children, changes the extent of intervention or avoids unnecessary procedures.

The most convincing data, for now, is the reduction of friction in the pathway: less sedation, less contrast, and shorter times without sacrificing the necessary information for decision-making. [1–3]
