# Discoid meniscus in children: symptoms and diagnosis

An unusually shaped meniscus may cause no problems. Pain, swelling or locking guide assessment and treatment.

Updated: October 2026 · Medically reviewed by Dr. Daniele Priano

A **discoid meniscus** is a variation in the shape of the fibrocartilage cushion between the thighbone and shinbone. It is wider than usual and mostly affects the outer side of the knee. It is present from birth, rather than being a change caused by sport.

It may never cause symptoms. An incidental finding without problems is usually observed: an MRI appearance alone is not a reason for surgery.

### A click does not establish the diagnosis

Pain, recurrent swelling, painful snapping, giving way or difficulty straightening the knee may occur if the meniscus tears or is unstable. Other knee problems can cause similar symptoms. A painless click without restriction is different from a knee that truly locks.

### What imaging adds

Examination assesses movement, pain and stability. X-rays do not directly show the meniscus, but can help identify other causes; MRI assesses its shape and possible tears. Findings must match the examination: not every signal change represents a tear needing repair.

### Observation, activity adjustment or surgery

For mild symptoms, the clinician may recommend temporary activity adjustment and work on movement and strength. Exercises do not change the shape of the meniscus. Persistent symptoms attributable to a tear, instability or locking may lead to discussion of [discoid meniscus surgery](https://ortopediaevolutiva.com/en/surgery/discoid-meniscus), aiming to preserve as much functional tissue as possible.

## Assessment and treatment

### When to seek evaluation
- Recurrent pain or swelling during activity
- Painful snapping, giving way or loss of full extension
- A locked knee needs prompt assessment; do not force it straight
- A hot, very swollen knee, fever or inability to bear weight needs urgent assessment rather than assuming it is the meniscus

### What is evaluated
- Duration of symptoms, any injury and actual limitations
- Knee movement, stability and location of pain
- X-rays and MRI when indicated; whether findings explain symptoms

### Treatment options
- Observation for an asymptomatic finding
- Individual activity management and physiotherapy when appropriate
- Arthroscopic assessment for persistent symptoms, a tear or clinically relevant instability
