# Köhler's Disease: Foot Pain in Young Children

A 3-7 year old limping with foot pain: Köhler's disease is an osteochondritis of the tarsal navicular.

Updated: September 2026 · Medically reviewed by [Dr. Daniele Priano](https://ortopediaevolutiva.com/en/about)

## Quick Answers

Does Köhler's disease heal completely? +

The outlook is generally favourable and the navicular gradually remodels, but clinical follow-up remains important. Symptoms often improve before X-rays return to normal, which may take years. Persistent or changing symptoms warrant reassessment.

Is a cast needed? +

Not always. Immobilisation can be useful when pain and limping are substantial, but the choice and duration depend on the clinical picture. Less symptomatic cases may need only activity modification and other comfort measures. A standard duration should not be treated as a prescription for every child.

A 4-5 year old who starts limping, prefers walking on the outer edge of the foot, and complains of a vague pain "on top of the foot" that doesn't go away with rest: this is the typical picture of **Köhler's disease**.

It's an osteonecrosis — a temporary vascular insufficiency — of the tarsal navicular, a small bone on the medial dorsal aspect of the foot. It affects children aged **3 to 7**, with a clear male predominance, and the exact cause isn't entirely clear: a combination of mechanical compression (the navicular is the last tarsal bone to ossify) and vascular factors is hypothesized.

The X-ray can look alarming to parents: the navicular appears fragmented, sclerotic, or flattened. But the **prognosis is generally favourable**: the navicular gradually remodels, often over years, and long-term functional problems are uncommon.

Treatment depends on symptom severity: activity is modified and comfort measures may help; more symptomatic cases may benefit from immobilisation. Protection, weight-bearing and duration are prescribed individually. Clinical improvement may precede normal X-rays, so return to activity is not determined solely by a date or cast removal.

## Assessment and treatment

### When to seek evaluation
- Pain at the medial dorsal foot
- Gradual onset limping
- Walking on the outer edge of the foot
- Localized swelling at the arch of the foot
- Tenderness over the tarsal navicular

### What is evaluated
- Palpation of the tarsal navicular
- Gait and limp assessment
- Foot X-ray (fragmented, sclerotic or flattened navicular)
- Differential diagnosis with fractures or infections
- Foot arch evaluation

### Treatment options
- Temporary reduction of activities that provoke pain and limping
- Supports, footwear or immobilisation may be selected in symptomatic cases according to pain severity and function
- Treatment mainly controls symptoms while the navicular remodels; there is no mandatory combination of orthosis, shoe and cast
- Reassessment if pain, swelling or limping does not follow the expected course or the presentation is atypical

How is it diagnosed? ▼

Age, pain location, limping and examination guide suspicion; X-rays may show typical navicular changes, but the finding must be interpreted in clinical context and against alternative diagnoses. An alarming-looking X-ray does not by itself determine severity or treatment need.

Will my child play sports normally? ▼

Usually yes, with a progressive return agreed after clinical recovery. Waiting years for completely normal X-rays is not automatically necessary: pain, walking, function and specialist assessment guide the plan. Alternative activities also depend on tolerance and permitted loading.
